IMMEDIATE IMPLANTS
An immediate implant will only be placed if the facial plate is intact; otherwise, we will utilize a two-step implant approach or delayed approach. An immediate implant is “implant in a day.” The tooth is taken out and an implant is placed on the same visit. However, the crown will have to wait approximately 2 months- we want to maintain the highest level of implant success and therefore do not want any pressure pushing down on the implant from a crown, either temporary or permanent.
With an immediate implant, a custom healing abutment will be fabricated to maintain the original soft tissue gingival architecture. Additionally, connective tissue or allograft may be used on the facial to increase tissue thickness for maximum esthetics. In addition, we will fabricate a custom impression coping for the patient to bring you when the implant is ready for restoration.
DELAYED IMPLANTS
A delayed implant or a two-stage implant is the traditional method of implant placement. It involves the extraction of the tooth with or without grafting, letting the site heal, and then proceeding with implant placement. This method is generally preferred due to the presence of a solid ridge of bone at the time of implant placement (versus placing an implant into a fresh extraction socket). This is our technique when dealing with molar teeth. Implant success is an important part of our practice and we strive to minimize the risks involved with the immediate technique.

Patient presents with fractured tooth #9 at the gum line. An immediate implant is placed with a custom healing abutment. Following 2-3 months healing, the final restoration is placed, maintaining the previous gingival architecture.

The patient previously had severe recession and bone loss around tooth #9. A free gingival graft was used to treat the recession defect along with orthodontic extrusion. At the time of implant placement, an immediate temporary was fabricated using the patient’s own tooth.
